Modernising Medical Careers (MMC) is a broad policy statement from the four UK Health Ministers that was published in February 2003. It sets a new direction for postgraduate medical education, reflecting the formal consultation on the proposals originally set out in Unfinished Business: reforming the SHO grade. The Operational Framework for Foundation Training has been developed to help support the implementation and delivery of foundation training that will commence across the UK in August 2005. From that date it is anticipated that all UK medical graduates will undertake a two-year Foundation Training Programme.

As the Modernising Medical Careers programme does not include dentistry, there will be a continued need for senior house officer (SHO) posts in the dental specialties after the introduction of Foundation Year 1 and 2 posts to medicine. Professor Sir John Tooke led an inquiry into Modernising Medical Careers (MMC) that culminated in a final report, Aspiring to Excellence, published in January 2008. The Secretary of State for Health gave his formal response to that report the following month. Subsequent to those publications, the Parliamentary Health Select Committee (HSC) published the findings of their own inquiry into MMC in May. One of the HSC recommendations was that the Department of Health should publish an updated response to the 47 recommendations in Sir John's original report. The Department agreed to that HSC recommendation and our updated response is published here.

These proposals have been developed as the result of detailed discussions with nearly 3,000 stakeholders. This UK initiative will be taken forward by the four countries in partnership with all major stakeholders, including patient groups, to ensure that the healthcare science workforce is fit for the future in a rapidly changing and evolving healthcare environment.
