There is loads of work being done at local level to tackle the problem of delayed transfers of care.

We've pulled together some 'bright ideas' that might be helpful in finding solutions to the challenges your own organisation faces.

These initiatives are all at different stages and not all have them have been thoroughly tried and tested yet, so we can't really recommend any in particular. But it's worth taking a look at what has worked for other people. We've given their contact details so you can talk to them direct if you want more information.
