Summary of the CAT's work

Introduction

Since its inception in 2002, the Change Agent Team (CAT) has developed a range of programmes and products to help health and social care organisations reduce delayed transfers of care by improving services for older people.
This guide has been prepared to help Strategic Health Authorities, NHS organisations and local authorities understand what help is available and make it easier to access this by setting out a 'menu' of support options.
In offering support, the Team works closely with different parts of the Modernisation Agency to prevent duplication and ensure that support is coherent and focused.
A central feature of our approach is to support sustainable change through a whole systems approach - this is as much about people, professions and skills as it is about beds and buildings. Hence much of the support we offer is about partnership working across- as well as within - organisations.
