Under the Community Care Act, hospitals must notify social services each time they anticipate a patient will need community care on discharge. They must also notify social services of proposed discharge dates, to ensure both parties are clear about when the reimbursement period will start.

  • Model Notification: planning for the implementation of reimbursement and improving hospital discharge practice
  • Notification Flowchart
  • Example notification system - model notifications based on faxes
  • Reimbursement - the timing of issuing notices - This document sets out the timing around issuing assessment and discharge notifications and gives examples of when reimbursement charging can start following the issue of notices
