Integrated care
Integrated care is when health and social care services work together to ensure individuals get the right treatment and care they need for their health concerns
Piloting new models of integrated care
From 2 July 2009, the 16 Integrated Care Pilots (ICP), announced by the Department of Health on 1 April this year, are beginning their innovations in the integration of service provision. This is part of a two-year programme, transforming the way people experience health and social care.
