Neuro-behavioural Rehabilitation Unit (NRU), Royal Hospital for Neuro-disability

The Neuro-behavioural Rehabilitation Unit (NRU) is a national inpatient service specialising in assessment, rehabilitation and long-term disability management of adults with neuro-disability.

Having a neuro-disability can be compounded by displaying challenging behaviour, which is a consequence of acquired brain injury. The unit admits individuals whose challenging behaviour is preventing them from participating in rehabilitation or onward placement. The goal of the unit is to enable each individual to fulfil their potential for rehabilitation and disability management whilst enhancing their quality of life.

The Unit follows a philosophy of 24-hour behaviour management using 'positive programming' ie aversive strategies such as time out or response cost are not used. Each patient has an individualised Behaviour Support Plan which is based on extensive assessment and functional analysis. Proactive strategies include environmental management, positive programming and focused behavioural support.

The Unit  aims to:

  • implement programmes designed to reduce the frequency of inappropriate behaviour and monitor such programmes for effectiveness;
  • provide therapeutic programmes that promote mobility and physical function, increase cognitive and social skills and improve physical and mental health;
  • enhance leisure and recreational activities;
  • provide education and support for carers
  • ensure a seamless service right through to the individual being discharged and given a placement.

The Unit has developed a variety of approaches and tools which it is willing to share:

  • behavioural assessment incorporating the Challenging Behaviour Inventory and Functional Analysis
  • Behaviour Support Plan
  • positive programming approach to neuro-behavioural rehabilitation;
  • verbal de-escalation training;
  • risk assessment;
  • functional focussed goal setting;
  • interdisciplinary working practice;
  • Triad model (key therapist, key nurse, key healthcare assistant)
