Community Neuro-disability Unit, The Royal Hospital for Neuro-disability, London

The Community Neuro-disability Unit meets the disability and nursing needs of people with profound neuro-disabilities in the community and helps them to remain at home. The service works in partnership with primary health, social care and other agencies. Care provision is seamless so that people can move from one service to another dependent upon their needs.

Services include:

  • community assessment service with day attendance or short-term admission;
  • rehabilitation for people who are slow to recover to improve function, skills and secondary health conditions;
  • disability management to prevent deterioration in function (including secondary health needs) and consequent admission to long-term care and
  • health management for stabilisation and improvement of health status in order to prevent the break down of community packages of care.

An audit of the service showed that it has prevented admission to acute units and long-term care settings by meeting the secondary health needs of people with profound neuro-disability.
