Independent Living Team, Nottingham

The Independent Living Team is a community based, city-wide service, and provides a care-managed system of assessment, care planning, monitoring, reviewing and rehabilitation. Services commissioned by the team include home care, day care, Direct Payments, residential care and respite care. The team also ensures that individuals maximise their own incomes through welfare rights information, and accessing the Independent Living Fund.

The team is willing to share their experiences with any organisation with similar aims. They have skills in multidisciplinary working, joint protocols and procedures, development of experimental living flats and of specialist private home care provision, and expertise in the delivery of Direct Payments.
