Optua is a leading disability organisation in Suffolk, providing a range of services to disabled people. Rethink aims to give disabled people opportunities to play an active part in the community and to recognise their right to equal participation in every aspect of life.
They provide the following services:
- supported housing (working with a housing provider to provide purpose-built housing with 24-hour support as required);
- outreach support (tailor-made support packages delivered to clients already living in their own home);
- 'Move-on' unit (a specialist purpose-built flat to provide safe environment for assessment of people with cognitive problems in the community, with support available up to 24 hours per day) and
- case management (every client has a case manager co-ordinating their care to ensure that all their needs are met).
Optua have produced various tools, including case studies and costing papers which they are willing to share.
