The Neurology Outreach Therapy Service is a multidisciplinary service for people with any neurological condition and their families, that continues to serve individuals in the community after they have been diagnosed.
The service aims to enable people to:
- maintain independence (despite having a deteriorating condition)
- and achieve control (even when the condition is rapidly deteriorating).
This is achieved by:
- providing specialist neurological physiotherapy and occupational therapy at a place of the person's choice, e.g. hospital or community;
- decreasing stays in hospital;
- providing the link between primary and secondary care and having one point of contact to make accessing services easier;
- providing high quality, easily accessible information and education to individuals, carers and healthcare professionals;
- co-ordinating care and ensuring appropriate intervention where necessary
- providing assessment, (both physical and functional) in order to promote maximum independence and quality of life and
- providing therapy as part of weekly multiple sclerosis and neuro-muscular clinics.
The service has developed a Wellness Programme for people newly diagnosed with multiple sclerosis.
