Southwark Primary Care Trust, Multiple Sclerosis Nursing Service

The Multiple Sclerosis (MS) Nursing Service at Southwark PCT provides support, information and advice to people living with MS. The service supports people with MS from diagnosis through to end of life.

There is an open referral system, and individuals are offered a choice of appointment location eg to be seen in their own homes, in the outpatient department, or in community clinics.

The service has produced templates for:

  • establishing and running a ‘newly diagnosed’ group session;
  • an open referral system;
  • a “fast-track” relapse service and management protocol, which ensures a rapid responsive service for people who have a relapse of their MS;
  • care pathway for newly-diagnosed patients commencing disease modifying therapies and
  • care pathway for patients who have become pregnant.
