Call for examples of good practice in COPD

As we are developing the national strategies for COPD we are looking for examples of best (or good) practice and innovation.This theme will run throughout the strategy, with each work stream containing examples of different approaches and innovative ways of working from both health and social care.
We would therefore like to hear of any examples that relate to the provision of services for people with COPD and their carers.
The examples should demonstrate:
- Support for implementation of the COPD NSF
- Making a positive difference to people living with COPD
- The following are examples of areas of interest:
- Local and regional implementation arrangements
- Integrated health and social care assessment and care planning for people with COPD.
- Meeting the information need of people with COPD, e.g. information about local services, information and supprot to self-care such as expert programmes
- Development for care pathways for COPD
The examples should outline:-
- The aim or objective of the project
- A short description of the project
- Project outcomes
