The DH (Long Term Conditions policy team) have sponsored the Prince’s Foundation for Integrated Health Awards for England for the next three years
This award is to reward outstanding examples of how integrated health can make a real difference to people’s lives. They are for organisations who demonstrate innovative integrated approaches that take into account the factors that empower people to exercise more control over their health and wellbeing.
This approach fits in with the Departments vision for improving care for people with long term conditions through an integrated and holistic approach, which ultimately enables people to more independent, whilst improving there quality of life, health and well-being.
The 2008 Integrated Health Awards were launched on 23 June 2008 with a closing date for applications on 29 August 2008. The awards ceremony is due to take place early next year.

This aims to ensure that implementation of the quality requirements in the National Service Framework (NSF) for Long-Term Conditions is facilitated by the availability of the appropriate information for patients, families and carers, the general public and care professionals.
