The National Service Framework for long term conditions

This NSF sets 11 quality requirements to transform the way health and social care services support people with long-term neurological conditions to live as independently as possible. Although the NSF focuses on people with long-term neurological conditions, much of the guidance it offers can apply to anyone living with a long-term condition.

The National Service Framework for long-term conditions: information leaflet

This is a leaflet for the public which gives a summary of the key messages contained in the NSF for long-term vonditions. Copies will be placed in GPs' surgeries, hospitals and other sites where patient information is on hand.

The National Service Framework for long term conditions: Good Practice Guide and examples

The Good Practice Guide suggests how the Quality Requirements in the NSF can be implemented locally. It contains some evaluated examples of good practice provided by organisations willing to share their experiences of implementing NSF recommendations. The Good Practce Guide was updated in January 2006 and new examples of good practice added.

The National Service Framework for long term conditions: information strategy

The strategy ensures that the implementation of the quality requirements in the NSF is supported by appropriate information for people with long-term neurological conditions, families, carers, general public and care professionals.
