The Long-term (Neurological) Conditions National Service Framework (NSF) was launched in March 2005. The NSF aims to transform the way health and social care services support people to live with long-term neurological conditions. Key themes are independent living, care planned around the needs and choices of the individual, easier, timely access to services and joint working across all agencies and disciplines involved. The principles of the NSF are also relevant to service development for other long-term conditions. This NSF is a key tool for delivering the government's strategy to support people with long-term conditions outlined in the White Paper Our health, our care, our say and the NHS Improvement Plan: Putting People at the Heart of Public Services. It applies to health and social services working with local agencies involved in supporting people to live independently, such as providers of transport, housing, employment, education, benefits and pensions.
This document is the product of collaboration between DH, Care Service Improvement Partnership (CSIP) and the Third sector, and gives an overview of resources, tools and initiatives that are/will be available to support local implementation of the NSF.
The Care Services Improvement Partnership (CSIP) has produced the 10 Quick Wins poster and leaflet, which are key tools in implementing the NSF for Long-term Conditions. They focus on high priority and high impact areas where services can be improved without the need for additional funding and resources.

The NSF for Long-term (Neurological) Conditions Community website aims to help people with an interest in implementing the Long-Term Conditions NSF. It has three interlinked parts which work together to bring information about the NSF together in one place; to enable people to assess how they are doing, point them towards information and expertise to help them change and track their progress; and to allow a community of people with a common interest in long-term neurological conditions to work together, share ideas and knowledge and achieve more than they might as individuals.
