The original version of the QOF guidance and evidence base, which was published with the contract agreement in 2003, has been revised by the NHS Confederation and the BMA.

Updated version of original guidance

An explanatory note describing the changes is also provided. This does not change any of the QOF indicators, thresholds or points scores.

Annual QOF review process: guidance

This sets out the principles of how the annual QOF review visit process should work and should be read in conjunction with "Delivering Investment in General Practice: Implementing the New GMS Contract" and the supplementary documents to the June 2003 version of the contract. It has been agreed with the NHS Confederation and GPC.
