South Sefton aims to use the programme to develop a new practice delivering GP and other local health services for up to 9,000 patients. There will be a particular emphasis on making primary care services more accessible, reducing the need for local people to travel to hospital and improving the quality of life of people with long term conditions.

The practice will be expected to deliver three key levels of service:

  • A modern practice providing high quality family healthcare services to up to 9,000 patients
  • Active management of patients to improve patient experience and reduce the need to access hospital services - this will include coordinating health and social care for patients with long term conditions
  • Other services to follow around 18 months after the core services are established.  These will include phlebotomy (blood) services, a falls prevention service and pain management service, all of which will cater for patients from a wider area

In keeping with the PCT's key objectives, the new practice will also be expected to contribute to improving the overall health of local people by promoting public health and delivering high quality patient education.

The onus will be on the successful applicant to identify and set up premises for the new services, which could be a new-build facility or occupy space in an existing building.  The PCT will work closely with them to ensure both the building and location are suitable to meet the needs of local people.
