Letter from Anne-Marie Millar to Strategic Health Authority Directors of Finance to clarifiy how and where organisations should make disclosures or qualifications on their Statement on Internal Control about risk management and review processes being in place for the full year, gaps in assurance frameworks and significant control issues for 2004/2005.
This note sets out how and when disclosures or qualifications to the SIC should be made.
Since 2003/2004 there has been only one template for the SIC which all NHS bodies must sign (with the exception of certain Special Health Authorities who are using the Treasury Government Department model). A pro-forma SIC was attached to Sir Nigel Crisp's letter of 15 September 2003.
