A VMX circuit breaker failed catastrophically due to partial discharge activity caused by a tag on insulating tape compromising the clearance between the top and middle bus-bar. Restrict unit to remote operation only until visual inspection and the completion of any remedial work is carried out and verified by the Trust's Authorised Engineer (HV).
