An incident has been reported where within a few minutes of an OCB operating, to clear a fault in a CT chamber a further flashover occurred on the busbar bushings of the OCB truck. The OCB truck and housing were severely damaged and the rear door of the substation was blown open. After investigation, an amendment to this notice has been issued: see NHSE (2004) 09-1.
