Two incidents have occurred associated with a pharmacy chill store resulting in more than £70,000 worth of vaccines freezing due to inappropriate temperature controller default settings, rendering them unusable. Automatic temperature controllers in vaccine chill stores should be checked to ensure that the default settings are appropriate for the application in order to afford the maximum protection of the contents in the event of control system failure.
