The NHS Executive requires information on GOS sight tests and vouchers for a number of reasons, including informing consultations on the level of the GOS sight test fee, estimating the cost of parts of the service and forecasting expenditure. I would be grateful if you could arrange for the enclosed SBE 515 return to be completed for the six monthly period ending 31 March 1998 and returned no later than 15 April 1998.
