This notice announces: i: Phase in of revised GOS forms from 17 November 1997 ii. How the revised spectacle voucher form clarifies arrangements for the issue and completion of spectacle vouchers. The Annex provides further advice and clarification of 1305 procedures. It advises that in future all prescriptions issued under the GOS should be written in the highest spherical power.
