on page 21
- in the green table Advised Prescribing Regime, the right-hand column should be headed "Greater than 0.7ppm"
- in the specimen prescription in the bottom right it should be "(F-1 mg)"
on page 22 the specimen prescription in the bottom right should read "Sodium fluoride mouthwash 0.05%".
