This is a report on three investigations by the Health Service Ombudsman of complaints relating to the death of young women with diabetes. The Ombudsman investigated complaints against a GP, Halton General Hospital and the way that the Independent Review Panel convened by North Wales Health Authority had reached its conclusions about the care provided by the GP. The Ombudsman publishes the findings as a separate report as they illustrate lessons that may be learned about the management of a common condition. The report also aims to provide useful material for health professionals, educationalists and those involved in supporting and caring for people with diabetes.
