The 'Discharge from Hospital : Good Practice Checklist' produced by the Change Agent Team in Autumn 2002 has been widely used by Councils and their NHS partners to assess their discharge arrangements and identify changes required. The aim of this supplementary checklist is to help Councils and their NHS partners: develop a joint policy on patient choice during the discharge process; review their local policy and procedures in relation to the 'Choice of Accommodation'Directions and to identify what changes may be needed to improve existing arrangements, including the increased engagement of the independent sector.
