The form Certificate of Vision Impairment (CVI) is to replace form BD8 (1990) and the CVI form 2003 from 1 September 2005. This form formally certifies someone as partially sighted or as blind so that the local council can register him or her. Its second purpose is to record a standard range of diagnostic and other data that may be used for epidemiological analysis.
