This Statement of Financial Entitlements (SFE) relates to the payments to be made by primary care trusts (PCTs) to a contractor under a General Medical Services (GMS) contract. It replaces the Statement of Financial Entitlements for 2004/5, signed on 30 March 2004 and revised on 17 September 2004 and 30 March 2005, but the 2004/5 SFE continues to have effect in relation to claims for payments that relate to the financial year 2004 to 2005.

This document was updated on 8 June 2005, 1 February, 31 March, 1 July and 2 August 2006 and 25 September 2006.
