This exemplar describes a care pathway for the discharge and ongoing support needs of children using long term ventilation in the community.

The care pathway, for the discharge, management and community support for children with long term ventilation needs, describes three stages: (1) the discharge process; (2) living at home; and (3) growing up and transition.

This care pathway should be used in conjunction with From Hospital to Home: Guidelines on Discharge Management and Community Support Needs of Children using Long Term Ventilation published by Barnardos (June 2005) which contains core principles underpinning the organisation and delivery of care, hints tips and tools for adapting and using care pathways locally.

The following care pathway provides a commentary on a child's life stages and events, a cross reference to themes within the NSF for Children, and cross references to policies, evidence and links - which are provided within Barnardo's document From Hospital to Home.
