This is a literature review of the evidence base for transition from paediatric to adult health services.

This document is based on a presentation given at a major national conference on 23 March 2006 convened by the Department of Health, the Royal Colleges and the Department for Skills and Education. The aim of the conference was to raise awareness of this area and empower existing local champions of transition to support further development of this vital and important area with respect to clinical service provision, education and training in addition to research.
