This report presents information from the fourth year of the data collection from the Referrals, Assessments and Packages of Care (RAP) project for adults. Information presented here relates to England for the period 1 April 2003 to 31 March 2004. National level information only is given in this report, tables containing analysis at CSSR level are available for download below.
The Report contains a summary of key findings followed by 6 sections and 4 annexes. Sections 1 and 2 give some background to the RAP project, including a brief guide to each of the individual RAP proformas (full copies of these are available on the RAP website). Section 3 looks at the level of response to RAP in 2003-04 in terms of how well each of the RAP proformas were completed. Section 4 presents and comments on the main findings, mostly at national level. Reference tables are provided in Section 5. Section 6 discusses data quality issues for this and the earlier RAP exercises (1999-00 to 2002-03). Annex 1 contains a flowchart showing the relationship between the different RAP proformas. Annexes 2 and 3 list the basis of return and level of completion for each of the RAP proformas by CSSR, and annex 4 sets out the linkage of specific RAP returns to the PAF and Best Value indicators for 2003-04.
