The RAP data collection documentation is in two volumes, and constitutes a resource pack and set of return proformas for Referrals, Assessments and Packages of Care (RAP) relating to adult services for the period 1 April 2005 to 31 March 2006. Returns are requested from all Councils with Social Services Responsibilities (CSSRs).
