Community Care Statistics 2002-03: Referrals, Assessments and Packages of Care for Adults: Report of findings from the 2002-03 RAP collection, England, 1 April 2002 to 31 March 2003

This report presents information from the third proper year of the data collection from the Referrals, Assessments and Packages of Care (RAP) project for adults. Information presented here relates to England for the period 1 April 2002 to 31 March 2003.

Findings

Response

  • All 150 CSSRs provided a set of RAP proformas for 2002-03. However, only 46 councils completed all the 15 proformas and 54 councils did not provide any data for the forms asking for waiting times information from the contact day to the receipt of all the specified services in the care plan (proforma P6).

During the year 2002-2003, for England:

Referrals

  • An estimated 3.29 million screened contacts were made to CSSRs in England in 2002-03, a fall of 63,000 or 2% from a year ago.
  • Around 1.71 million of the screened contacts (52%) resulted in further assessment of need or the commissioning of ongoing service and 1.58 million contacts were attended to solely at or near the point of contact.
  • Of the 3.29 million referrals, 858,000 (26%) were self-referrals, 731,000 (22%) were referred from Secondary Health (e.g. hospital wards), 433,000 (13%) were referred by family, friends or neighbours. There were 356,000 (11%) referrals from primary/community health.

Assessments

  • There were an estimated 1.01 million clients for whom assessments were completed during 2002-03, a decrease of 1% from 2001-02.
  • In respect of waiting times for new clients aged 65 and over, about 19% had their assessment completed within 2 days of first contact. 48% of all new older clients were assessed within 2 weeks and a further 23% within 6 weeks. This is similar to the distribution in 2001-02.
  • 856 thousand reviews were carried out in 2002-03, 14% higher than in 2001-2002.
  • Of those clients with completed assessments, 41% of clients had some or all (new) services already started before their assessment was completed. It was intended to provide new services to a further 24% but these had not started before the assessment was completed.
  • Around 20% of clients were not offered or intended to be provided with new services upon completion of assessment. The remaining clients either were offered but declined services or received other services outside of social services, for example from health or housing services.
  • About 336,000 clients for whom assessments were completed were recorded as having informal carers during 2002-03, a rise of about 1% from 2001-02. The number of carer assessments carried out, either separately or jointly with their client, has increased by 21% since 2001-02 to 313,000. Of these, 58 thousand carers had a separate assessment from their client, a rise of around 50% from 2001-02.

Packages of care

  • There were an estimated 1.68 million clients receiving services in 2002-03, a rise of 3% from 2001-02. However, when former preserved rights residents are excluded the increase was about 0.5%. These were services provided, purchased, or supported by CSSRs during the year following a community care assessment.
  • Community-based services were provided to about 1.40 million clients during the year, 2% more than in 2001-02, accounting for 84% of all clients receiving services. Removal of former preserved rights clients did not affect the year on year comparison.
  • In 2002-03 an estimated 581,000 clients received home care, 400,000 clients received equipment and adaptations as a service following assessment, 384,000 received professional support (e.g. occupational therapy) and 277,000 received day care services. A client receiving more than one type of community based service will be included in each service received.
  • 9,600 people received direct payments, increasing from 6,300 in 2001-02, a rise of 53%.
  • Independent sector residential care services were provided to an estimated 218,000 clients. This is a rise of 12% from 2001-02 but when former preserved rights clients are excluded there was a fall of around 3%. An estimated 112,000 clients received nursing home care, up 5% on 2001-02 but a fall of about 3% when preserved rights clients are omitted. About 43,000 clients received Local Authority staffed residential care, a fall of around 15% since 2001-02.
  • An estimated 1.2 million, about 72%, of those receiving services following an assessment, were aged 65 and over.
  • Around 15% of new clients started to receive the first service of their care-package on the contact day or the following day. A further 29% waited up to two weeks for their first service following contact. Over two thirds of all new clients (71%) had received their first service within six weeks of their first contact compared to 68 percent in 2001-02.
