Referrals, assessments and packages of care: report of the first dress rehearsal England, 1 January 1999 to 31 March 1999

This report presents information from the first national data collection resulting from the Referrals, Assessment and Packages of Care project. The project was developed to provide a coherent set of national statistics on adult community care.
Findings
Response
104 or 69 per cent of Local Authorities provided a set of RAP proformas from the first dress rehearsal stage. The level of completion varied by the type of proforma, with those relating to 'assessments' being the least complete.
During the three month period, for England:
Referrals
- An estimated 965,000 screened contacts were made to Social Services Departments. 509,000 of these (or 53 per cent) resulted in further assessment of need or commissioning of ongoing service. 455,000 contacts were attended to solely at or near the point of contact.
- Of those where the type of service was known, 37 per cent of contacts attended to at or near the point of contact required information or advice only. Almost a quarter were concerned with orange badges, and just over a tenth were provided with welfare benefits advice.
- Of the total number of screened contacts, around 218,000 (23 per cent) were self-referrals, 159,000 (17 per cent) were referred from Secondary Health (e.g. hospital wards), 117,000 (12 per cent) were referred by family, friends or neighbours. There were 85,000 (9 per cent) referrals from primary/community health.
Assessments
- In all, there were an estimated 359,000 clients for whom assessments were completed or terminated during the three month period. Of these there were 312,000 clients for whom assessments were completed. Where information was available, 41 per cent of clients had some or all (new) services already started before their assessment was completed. Just under a quarter of clients were having no (new) services offered or intended to be started.
- About a fifth or 66,000 clients for whom assessments were completed were recorded as having informal carers.
- 58 per cent of assessments completed were for those aged 75 or more.
Packages of care
- There were an estimated 928,000 clients receiving services that were provided, purchased, or supported by Social Services Departments. Community-based services were provided to 851,000 clients. 368,000 clients received home care, 264,000 received professional support (e.g. occupational therapy) and 168,000 received day care services (it should be noted that a client may have had more than one type of service during the three month period).
- Independent sector residential care services were provided to an estimated 130,000 clients. An estimated 86,000 clients received nursing home care, with 54,000 receiving Local Authority staffed residential care.
- Just over half of clients receiving services were of the primary client type 'physical and sensory disability or frailty'.
- The vast majority (about 86 per cent) of those receiving either nursing care or LA staffed residential care were aged 65 or more. 73 per cent of those receiving independent residential care were aged 65 or more.
