NHS LifeCheck is a simple, easy-to-use health assessment questionnaire that will help people assess and manage their own health through the major life stages and beyond. The NHS LifeCheck programme is based upon a commitment in the 2006 ‘Our Health, Our Care, Our Say’ White paper to initially develop three NHS LifeChecks for early years, adolescent and mid-life. Since then, the Department of Health has been working with partners including health and social care professionals, academic researchers and experts and end-users to develop these three NHS LifeChecks.
The NHS Early Years LifeCheck is an online tool for parents and carers with babies 5-8 months old. It provides personalised information and useful advice about what parents can do to help keep their baby healthy, happy and safe. It has been developed by the Department of Health and the Department for Children, Schools and Families, in consultation with doctors, parents, nurses, midwives and health visitors, and is based on the Birth to Five strategy.

The NHS Mid-life LifeCheck is for people aged 45-60 and focuses on assessing lifestyle modifiable risk factors such as smoking, diet and nutrition, alcohol use, physical activity and emotional well-being. Using the information people provide, the NHS Mid-life LifeCheck tool provides users with a detailed and personalised assessment of their risk factors and then facilitates behavioural change by assisting the person in setting realistic and motivating personal goals, furthermore, supporting them to meet these goals with reminders and other motivational tools.
