An introduction to the Single Assessment Process (SAP) for older people
The standard aims to ensure that the NHS and social care services treat older people as individuals and enable them to make choices about their own care. This is achieved through SAP, integrated commissioning arrangements between health and social care and integrated provision of services.
The requirement to develop SAP followed recognition that many older people have wide-ranging welfare needs, and that agencies need to work together to ensure that assessment and subsequent care planning are effective and co-ordinated. Care should be holistic and involve service users.
