Final versions of National Framework decision support tools.

In conjunction with the implementation, on 1st October.  of the National Framework for Continuing Healthcare and NHS-funded nursing care we are publishing final versions of the decision support tool, the fast-track tool and the checklist (previously called the referral tool and the screening tool) which are available for download.
