Final versions of National Framework decision support tools.

In conjunction with the implementation on 1 October 2009 of the revised National Framework for Continuing Healthcare and NHS-funded nursing care we are publishing final versions of the decision support tool, the fast-track tool and the checklist (previously called the referral tool and the screening tool) which are available for download.  

Word versions of the checklist, decision support tool and the fast track tool can be found below. These are identical to the versions published on 27 July 2009. However these versions are in Word format so that practitioners carrying out assessments can directly type information into the tools.
